About Palliative Care Palliative care is care provided for people of all ages who have a life-limiting illness, with little or no prospect of cure, and for whom the primary treatment goal is quality of life.
About the Program The aim of the program is to help people with a life-limiting illness die at home if they want to.
The Program funds three nights of specialist nursing care for patients and families in the community. Referrals are made by palliative care services. The care is normally available to patients and families right at the end of life ¨C a time of significant change which leads to carer anxiety, stress and exhaustion. Without this essential nursing support, many people would need to be admitted to a hospital during the final days of their life.
